| | If you have had any prior surgery within the last 6-12 months on the body part about which you are consulting Chico Orthopaedic Surgery, you will need to furnish records of that surgery. Please complete and sign the Medical Records Release from Other Facilities form and send it to your physician.
Once we have received and reviewed the records, we will phone you to set an appointment for you to see Dr. Wilhite or Dr. Komas. | |
