Office Appointments

We try to schedule your appointment at a time convenient for you. Please tell the receptionist why you are coming in, so we can schedule the right amount of time for your visit. Note that your initital visit may take up to one hour, to allow time for a complete history and physical.

When you phone to make your appointment, please be prepared to provide your address, phone number, social security number, insurance information, and complete medication list.

If you need to cancel your appointment, please let us know at least 24 hours in advance by calling 891-6375 to speak with our receptionist or answering service. Failure to cancel at least 24 hours in advance will result in a missed appointment fee of $75 (click here for our No-Show Policy).
