ATTENTION ALL UNITED HEALTHCARE (UHC) & PACIFIC HEALTHCARE (PHC) PATIENTS
We regret to inform you that as of June 19th 2008, neither Dr. Wilhite nor Dr. Komas will be considered “In Network” providers for United Healthcare or Pacific Healthcare Insurance programs. Through exhausted efforts to renegotiate our present contract we have no other option but to terminate our present agreement. Therefore, all claims after this date will be processed “Out of Network”, resulting in a higher out of pocket expense. Financial counseling will be offered as we do for all of our patients regardless of insurance status.
We apologize for the inconvenience this may cause and encourage you to notify your human resource dept. or UHC & PHC directly.
